Step 2 – Complete the Chain of Custody Form (C.C.F.)
Verify you have the correct donor by viewing a valid photo ID
Fill in the areas highlighted in YELLOW (in real CCF it is no any highlights).
Don’t forget to complete donor name and ID (it could be any number) or SS number.
Mark Head or Body Hair (DO NOT MIX HAIR TYPES!)
