Step 7 – Final steps in the specimen packaging

- Verify Chain of Custody Form is completed
- Insert the Hair Specimen Envelope in the Biohazard Specimen Bag (small pocket)
- Take the top copy (white) and insert it into the Specimen Bag (large pocket)
- Seal the Specimen Bag – it is ready to ship
Step 8 – Send Sealed Specimen Bag
- Put sealed biohazard specimen collection bag into the preadressed envelope.
- The specimen can be sent via U.S. Mail, FedEx, UPS or other courier service to our laboratory.
